Founded 2011 by MG Alan B. Salisbury (USA, Ret.) and based in Alexandria, VA. The Case Coordination Program provides intensive, long-term, one-on-one case management for service members, veterans, and families facing the most complex and compounding situations, including those at risk of suicide and other negative outcomes. Caseworkers coordinate across multiple providers and stay with the case rather than making a single referral and closing the file, which is what separates this from a directory or a warm handoff. Signed a Memorandum of Understanding with the VA Veterans Experience Office in December 2018. Also operates PATRIOTlink, listed separately under Navigation and Coordination.
